Your primary care physician is your partner in health care and will guide you to the preventive care services that are right for you. If you have an HMO health plan, he or she will refer you to specialists, therapists, and hospitals. This coordinated care system keeps your team of health care providers on track and in communication. Better still, in addition to your Primary Care Physician and Specialists you will have access to advanced technologies and comprehensive preventive care services.

Your Health Care Advocates

DHMN Patient Care Coordinators are your personal health care experts. They are available to answer any questions, in English or in Spanish, about eligibility, your benefits, billing, appointments, or even help you select a physician that is a good fit for you. Our Patient Care Coordinators are located right in Santa Cruz and many have been with Dignity Health Medical Network for more than fifteen years. Give them a call between 8:30 am – 5 pm Monday – Friday.

A durable power of attorney is one that allows the person you appoint to act while you are incapacitated. Durable powers of attorney are vital in health care situations because they let your agent make decisions when you are too sick to do so.

PEP classes

Dominican Hospital offers a variety of educational programs, health seminars and exercise and wellness classes. Please check out the online catalog to find a class that meet your specific needs.

Some classes and programs that are either free or discounted for Dignity Health Medical Network (DHMN) patients who are patients of the following HMO’s: Aetna, Anthem Blue Cross, Blue Shield, Cigna, Health Net, Great West Healthcare, UnitedHealthcare, and SecureHorizons. Registration and a physician’s referral are required. Proof of HMO insurance must be presented at time of registration. Dignity Health Medical Network (DHMN) will make clinical and payment decisions based on medical necessity, appropriateness of care and the benefits available to our patients. There are no financial incentives or payments made to DHMN staff that encourage or discourage the denial or approval of medical services.
